Aspects of the design of the WISDAM record.

A. Type of Folder

Photos and details of different options.

Need for a basic version for the essential core of no more than 5-10 pages

but with the option, for more complex records, to substitute this with a larger version allowing 20-50 pages.

B. Outside Front Cover

Aim: Simplicity. Clarity


In the UK both the NHS and the Department of Social Security Logos can be on the front page

but these should never, ever, be top right

since, to reduce human error, in every care or medical record

the person's name etc. should always be top right

a) Likes to be called Large Print to be read easily even by the partially sighted

b) Family Name Also Large Print to be read easily even by the partially sighted

Omit if “Likes to be called” involves the Family Name, for example, if someone likes to be called “Mrs Dayze” rather than "Pam" or "Pamela"

c) Age in smaller print

C. Inside Front Cover

Place to keep loose items.

In particular a place to keep the most recent version of repeat prescription requests


In time it is hoped that all such repeat prescription requests will have a QR code

allowing any smart phone to input the data into any relevant computer system e.g. An A & E system

D. Main Page Proforma or Printout

Printout of all currently documented Sinbads together with QR Codes.

This should always include the following:
